PD Application
Before completing the Project Discovery Student Application, you must first register and then logon to the Project Discovery site. Once registered and logged on to the Project Discovery site you will have access to the online Student Application and Medical Release form.
The Student Application has 5 sections. You may want to download the PDF version prior to completing the online form.
Upon completion of the Student Application and Medical Release Form, return to this page and click the PDF icon to print your completed application so that you may sign and date the forms. Give this completed application to you coordinator.as You may return to this page at anytime to download your completed application.
Project Discovery will notify you once your signed application has been received and processed.
