Student Information
There are five sections to the Project Discovery Student Application:
- Student Information
- Family Information
- School Information
- Discovery Information
- Follow up and Permissions
Complete each page and click Continue to Confirm information entered. From the Confirmation page, you may edit information or Continue to the next section. NOTE: you must complete the entire application and click Save for Project Discovery to receive your application.
After you Save your application, the final page will give you the opportunity to print your application. You must print the application, sign, and return to Project Discovery.
All information given on this application is strictly confidential and will not be released without a parent's written commission.
